Important: If you haven’t completed your forms online, you’ll need to fill them out at the clinic, including the Authorization and Consent for Treatment form.

Preparing for Your Appointment

Please confirm at least 24 hours in advance via our secure text or email reminders.

To minimize wait times, complete your pre-registration online before arriving. A secure link will be sent via text or email—simply follow the instructions to provide necessary details and health history.

To avoid delays in care, please arrive at least 15 minutes before your scheduled appointment to complete registration before meeting with your provider.

Running Late or Need to Reschedule?

If you’re running more than 5 minutes late, please call our office immediately. While we will do our best to accommodate you, wait times may apply, or we may need to reschedule to keep appointments running on time.

If you need to cancel or reschedule, please notify us at least 24 hours in advance to avoid a no-show fee.

Important: Repeated no-shows may affect your ability to schedule future appointments.
